all neutrik (the best!)
XLR's -black w/ gold connectors, right angle 3 pin - NC3MRC-B (male),
NC3FRC-B (female)
right angle 4 pin female silver - NC4FRC (no need for gold connectors on a
power cable!)
a pain in the ass to solder, but well worth it. they are strong enough that
your gear can be placed vertically, with the cables on the bottom, I have my
v3 and ad2k this way, but it would be really key for the apoge products
which are sop much deeper in design.

In case b and c: yes.
But directly with AD and DA-conversion, no monitoring over tape.

I don´t know such a card.

The only low profile XLRs I've ever seen were mini xlrs made by
Switchcraft.
The are not compatible with full size XLR connectors though.
You can purchase right angle XLR connectors from Markertek.

I have a used DAP1 in excellent working condition. Some superficial
blemishes, but nothing detracting. It comes with the wall wart with which I
modified with a 4pin connector that you can disconnect the power cable from
the wall wart and use with an echocharge or other 4 pin SLA. I used a
homebrew with it. It also has the NiCad internal battery that came with it
but it does not hold a charge. You can have the cells replaced for
relatively cheap if you wish to use it. I'm looking for $700.
