Hi, all.  Is there a CD storage solution  which: 
Is 3 ring loose-leaf binder
Holds 4 discs per page
Holds the  front CD insert in the same pocket as the CD?

Have been using Case Logic,  but the binders don't do 3 ring, and the 
ProSleeves don't hold the CD  inserts..
